IL23A(Interleukin-23 subunit alpha)是白细胞介素23(IL-23)的一个关键亚基,与IL12B(Interleukin-12 subunit beta)共同组成功能性细胞因子IL-23。IL-23主要由活化的树突细胞和巨噬细胞分泌,属于IL-12细胞因子家族。该家族成员(如IL-12、IL-23、IL-27和IL-35)均以异源二聚体形式存在,通过调节T细胞和自然杀伤细胞(NK细胞)的免疫反应参与炎症和抗感染过程。IL-23的主要作用位点是免疫细胞表面的IL-23受体(IL-23R),其信号通路通过激活JAK-STAT(Janus激酶-信号转导和转录激活因子)途径,促进Th17细胞(辅助性T细胞17亚群)的分化和维持,进而分泌IL-17等促炎因子。IL23A的功能异常与多种自身免疫性疾病密切相关,例如银屑病(牛皮癣)、炎症性肠病(如克罗恩病)和多发性硬化症。IL23A基因突变可能导致IL-23蛋白结构或表达异常,削弱其免疫调节功能,或引发过度炎症反应。若IL23A过表达,会增强Th17细胞活性,导致促炎因子(如IL-17、IL-22)过量分泌,加剧组织损伤和自身免疫疾病;反之,若表达降低,可能削弱对病原体的防御能力,但可能缓解炎症性疾病症状。针对IL-23/Th17通路的生物制剂(如乌司奴单抗Ustekinumab)已用于治疗银屑病和克罗恩病。IL-23家族成员的共性是调控适应性免疫和炎症反应,但各成员的具体功能存在差异,例如IL-12偏向诱导Th1细胞反应,而IL-23主要维持Th17细胞功能。

This gene encodes a subunit of the heterodimeric cytokine interleukin 23 (IL23). IL23 is composed of this protein and the p40 subunit of interleukin 12 (IL12B). The receptor of IL23 is formed by the beta 1 subunit of IL12 (IL12RB1) and an IL23 specific subunit, IL23R. Both IL23 and IL12 can activate the transcription activator STAT4, and stimulate the production of interferon-gamma (IFNG). In contrast to IL12, which acts mainly on naive CD4(+) T cells, IL23 preferentially acts on memory CD4(+) T cells. [provided by RefSeq, Jul 2008]
