IRAK4(白细胞介素-1受体相关激酶4)是IRAK家族的重要成员,属于丝氨酸/苏氨酸蛋白激酶,在先天免疫系统的Toll样受体(TLR)和白细胞介素-1受体(IL-1R)信号通路中起核心作用。IRAK家族包括IRAK1、IRAK2、IRAK3(IRAK-M)和IRAK4,它们共同参与调控炎症反应和免疫应答。IRAK4位于信号通路最上游,当TLR或IL-1R被激活后,IRAK4通过髓样分化因子88(MyD88)被招募到受体复合物上,磷酸化并激活下游IRAK1和IRAK2,进而触发NF-κB和MAPK信号通路,诱导促炎细胞因子(如TNF-α、IL-6)的产生。IRAK4的功能依赖于其激酶活性,但也可以作为支架蛋白发挥作用。IRAK4基因突变会导致其功能丧失,引发罕见的原发性免疫缺陷病,如IRAK4缺陷症,患者易反复发生化脓性细菌感染,但对病毒和真菌的抵抗力相对正常。IRAK4过表达与多种自身免疫性疾病(如类风湿关节炎、系统性红斑狼疮)和慢性炎症性疾病相关,可能通过过度激活NF-κB通路导致炎症因子过量产生。相反,IRAK4表达降低或抑制其活性可减轻炎症反应,因此IRAK4成为治疗炎症性疾病和自身免疫病的潜在靶点,已有小分子抑制剂进入临床试验阶段。IRAK家族蛋白均含有死亡结构域和激酶结构域,但IRAK3缺乏激酶活性,主要起负调控作用。IRAK4在进化上高度保守,其功能不可或缺,是连接病原体识别与免疫应答的关键分子。

This gene encodes a kinase that activates NF-kappaB in both the Toll-like receptor (TLR) and T-cell receptor (TCR) signaling pathways. The protein is essential for most innate immune responses. Mutations in this gene result in IRAK4 deficiency and recurrent invasive pneumococcal disease. Multiple transcript variants encoding different isoforms have been found for this gene.
