Intraductal oncocytic papillary neoplasm (IOPN) is a digestive system tumor typically found in the head of the pancreas. Most often, they remain confined to the pancreas; it is rare that they develop into an invasive carcinoma or metastasize. This paper presents the case of a 63-year-old postmenopausal female with a past medical history notable for BRCA1 mutation and exploratory laparotomy with pancreatoduodenectomy who underwent risk-reducing laparoscopic bilateral salpingo-oophorectomy. Postoperative pathology revealed metastatic oncocytic papillary neoplasm involving the bilateral ovaries. She was ultimately referred to gastrointestinal medical oncology for further management. This is the first report to our knowledge of pancreatic IOPN metastasizing to the ovaries. It is essential for cases to be reported for guidance of treatment regimens.
